Scenario

Aroha Rongi is a 73‐year‐old Maori lady who lives with her two daughters in Whatawhata. She complains of abdominal pain and nausea that have been getting worse over the past few weeks. Her daughter mentions that Aroha suffers from rheumatoid arthritis and diabetes, and regularly takes over‐the‐counter (OTC) ibuprofen and uses capsaicin (Zostrix) cream for her joint pain. She buys her analgesics from the supermarket as they are cheaper and easy to get than visiting her GP. Aroha visits her GP today because she has frequent dark and tar‐like bowel motions and a poor appetite. Her GP states that he is concerned about GI tract bleeding and asks Aroha to have an endoscopy done. Aroha declines to have an endoscopy and decides to continue with complimentary Mirimiri and Rongoā (Karamu, Kawakawa, and Kūmarahou).

• Smokes 1 pack per day for more than 40 years
• Anaemia
