NUR3117A Assessment Guidelines And Criteria

Component 1: Class active participation (10%)

Students will be assessed based on their participation in tutorial activities from week 0 to week 12. The activities involve participating in group discussions and presenting findings to the class.

Component 2: Group Project/Presentation Guidelines- Community Assessment (30%)

(25% Group work + 5% individual delivery)

 DUE Sunday 5 Oct 2359 hrs submit via CANVAS

a. Students will form small groups (4-6 students per group, choose 1 group leader) and perform community assessment and profiling.

b. Duration: 15 minutes presentation with 5 minutes for questions and discussion following the presentation. Total time (maximum) 20 minutes per group

c. You will select a geographic community of interest to your group

– A community can only be assessed by one group of students.  Once your group has made a decision, have a group leader notify the tutor via email.

d. Read and follow steps in e-book pg. 102-111 to conduct community health assessment and present the followings:

Introduction

– Describe your community’s profile/history and characteristics with supported evidence such as news, articles, social media, graphs, etc.
– State reasons of selecting the community your group chose.
Community Assessment– Critically analyse the observations, windshield survey or foot surveys, and interview findings to identify the health issues and the needs of the community i.e., social demographics, health inequalities, psychosocial issues, determinants of health, financial, resources, etc.
– Discuss the challenges your group faced during the community assessment process o Identification of Health Issues
– Based on your analysis, identify at least 3 community health issues/risks i.e., chronic disease, physical or mental conditions and justify the ONE most significant issue with evidence from literature to support analysis

e. PowerPoint is the format for presentation (# of slides doesn’t matter).

f. Presentation will be delivered face to face. Every student in the group must participate in the presentation.

g. Marks (5%) will be allocated for individual delivery and presentation skills.

h. Discussion should be generated following each presentation. Group engagement (Kahoot, survey, etc) is expected with preparation of questions and comments.

i. Students who are absent for the presentation without approval from the Course Lead or late submission (submitted after October 5th 2359) will be subjected to PENALTY MARKS for the assessment.

Component 3: Individual Writing Assignment – Community Nursing Plan of Care Management (60%)

  • This individual written assignment aims to consolidate the student’s learning by linking theory to clinical practice in the community. The community assessment experience provides students with real life physical and cognitive experiences, assesses community needs and identifies key health issues.
  • Using the findings from your group’s community assessment done, develop strategies, integrating a health promotion model/ theory, to address health/psychosocial issues from a case study in below. Apply your group’s chosen community’s resources and settings to develop individual/family nursing care plan to the issues.
    Develop a case study with your group’s identified health issue (at least 1 health issue) and address the questions below:

a. Identify and critically analyse at least 3 key health or psychosocial behaviour/issues affecting the client and/caregiver. Provide evidence from literature to support analysis.

b. Develop a home care self-management plan for 3 issues that were discussed in above by applying a health promotion model (e.g. HBM, TTM, or CCM). The plan should have specific goals/measurable outcomes and interventions to empower the client/caregivers to self-manage in the home/community. Provide evidences to support your proposed interventions. If your case-study has a patient being discharged from the hospital to home, you can also discuss the anticipated needs in the home that the patient has such as, equipment, services, and referrals.

c. In the plan, apply how existing community resources can be integrated into the management of care to deliver effective person-centered care in the community. The community area your group chose for the community assessment project will be considered as the community for the case-study.

d. Discuss evaluation strategy for assessing the measurable outcomes.

  • A case study can be developed from a resident your group encountered during the community assessment project or you can use your any family member/relative/friends/previous clinical attachment patient (without names) with chronic diseases will be acceptable (please see an example in below). Include caregivers and psychosocial issues.
  • Only assignments submitted in EXAMSOFT will be marked. If you submit the final document into Canvas, it will not be marked and late submission policy will be affected.   • 150-200 of words, must include chronic diseases, demographics, social/medical history, family support, health behavior issues. 

CASE- Scenario Example:   

Mr Tan is a married 70-year-old, retired Chinese gentleman and lives in a 2-room Flexi Flat with his wife aged 68 years old in Queenstown. They do not have any children. Mr Tan has frequent admissions to the hospital due to multiple episodes of breathlessness. He is a chronic smoker (started in the last 40 years) and he has medical history of hypertension diabetes and hyperlipidemia. In his recent hospital admission, Mr Tan was referred to the Respiratory Medicine department as he experienced persistent shortness of breath despite of inpatient treatment. Mr Tan’s spirometry test suggested of moderate to severe obstruction of airways. His primary diagnosis for admission concluded as Chronic Obstructive Pulmonary Disease (COPD) Stage 2. He was discharged with Salbutamol (Metered Dose Inhaler with Spacer). Prior to discharge, he received patient education on the use and effects of the inhalers, reinforcements of early symptom recognition and management and advice on smoking cessation.

  • Criteria:

–          Explore how existing community resources from your community assessment can be integrated into the management of care to deliver effective person-centered care in the community.

–          Discuss realistic evaluation strategy for each measurable outcomes.

 Quality of Evidence (10%) –           Support with high quality scholarly evidences.  –    Include an extensive literature review and        evidence based practice articles (at least 12).
 Clarity (10%) –           Clarify the main ideas and be consistent with the        flow of information.
 Use of Reference (10%) –     Adherence to APA 7th edition format.
  • The word limit for this assignment is 1,800 words (+/- 10%) excluding cover/title page, contents page, tables, figures, graphs, end-reference list and appendices.
  • High quality scholarly evidences are considered as peer-reviewed articles/books.
  • In-text citation and reference list should follow the 7th edition of the Publication Manual of the American Psychological Association (APA) style.
  • The marking criteria is attached in Appendix B in the assessment details.
  • Contents in tables will not be marked.
