Assignment Task

Introduction 

This assignment requires you to demonstrate your ability to critically analyse health, safety and wellbeing information to identify potential injury and/or health, safety and wellbeing issues in a workplace scenario.

You are also required to design and propose intervention programme strategies to reduce the incidence and severity of these issues, providing a rationale for your approach.

Intervention programmes must demonstrate a multi-layered approach that goes beyond individual awareness or competency alone.  This means combining individual-level actions (e.g. training and awareness) with organisational, environmental, or system-level controls (e.g. changes to work design, equipment, procedures, supervision, or support systems).  For example, training workers to use equipment safely must be supported by appropriate equipment selection, safe operating procedures, supervision and monitoring processes.

Important Note : The selected health, safety and wellbeing issues analysed in this assignment must relate to high risk activities, where credible harm outcomes include serious injury, occupational illness or fatality, as recognised in industry guidance or regulatory data.

For example, but not limited to: falls from height, operating heavy machinery, working with hazardous substances, lone working in remote areas, machinery entanglement or crushing, psychosocial risk leading to injury or illness, fatigue-related incidents, complex manual handling with high-force/highfrequency exposure leading to musculoskeletal disorders, confined space work, electrical work, and demolition that all require strict controls due to the high likelihood or severe consequence of injury or illness.

Low-risk, short-duration, or easily controlled hazards (e.g. wet floors, basic housekeeping issues) are not sufficient to meet the requirements of this assignment.

Intervention programmes that rely primarily on worker behaviour change without addressing organisational or system contributors will not meet the requirements of this assignment.

At least one of the identified issues referenced in this assignment must be supported by incident, injury, or near-miss data either from the chosen organisation, industry statistics, or regulatory publications.

Assignment requirements

Part 1:  Critically analyse occupational health and occupational safety information and tools used to identify accidents, incidents, injury, near-misses and future risk. 

1. Select a specific workplace/industry (e.g. construction, healthcare, manufacturing, office environments). This must be a different context to the one chosen in A1.
2. Describe the chosen context, focusing on key operational activities and known risks.
3. Identify at least three credible sources of health, safety and wellbeing information relevant to your chosen workplace/industry (1 each for health, safety and wellbeing).
4. Use at least one risk identification/analysis tool (e.g. Bowtie, Risk Matrix, 5 Whys, Fishbone, JSA, trend analysis) and apply it meaningfully to interpret your information and identify future risk.
5. Critically analyse the information to identify potential injury and/or health, safety and wellbeing issues in your workplace/industry context. Include a rationale for your selection. 

Part 2:  Formulate intervention programme strategies to reduce the incidence and severity of accidents, incidents, injury and near-misses and potential risk. 

Using the information obtained in Part 1:

1. Develop two (2) intervention programme strategies to reduce the incidence and severity of injury and/or health, safety and wellbeing issues in your chosen context.  Each intended programme must be proportionate to the high level of risk being addressed, and must be multi-layered.  For example, an intervention programme may include an awareness campaign, a training programme for safe equipment use, and a stress management and mental health support initiative. A training programme or awareness campaign alone is insufficient to meet the requirements of this assignment.
2. For each intervention programme include goals and objectives, a step by step implementation plan, and also monitoring and evaluation methods and processes that you will use to assess the effectiveness of the intervention programme.
3. Intervention programme strategies must demonstrate how both likelihood and consequence reduction for the identified high-risks, be proportionate to the level of harm being addressed, and how this reduction will be measured.
