Outline of the task

You will create a Portfolio (individual) with SMART objectives relevant to roles and responsibilities in a Health and Social Care setting. These targets are to be negotiated with the academic team and/or your potential/current employer, you will demonstrate achievement of goals and competence in a ‘work- based learning project’ which has been agreed as suitable to the Health and Social Care environment.

If you are not currently working in the Healthcare Sector you can base your Portfolio on the following:

  • GBS Community Educational Institute
  • Local community

Whichever option you choose you need to consider a product you can create to improve an area of practice. You will reflect on how this will develop your leadership Your assessment will be submitted as ONE submission, which will include the Learning Agreement.

Suggested Framework to structure the task

For this assignment, you will design and complete a project that produces a product relevant to your colleagues, community, or service users within your workplace, or another appropriate context such as your local or GBS community. The project will focus on developing your individual competence and leadership qualities through the setting and achievement of SMART goals.

In this assignment, you will:

  • Introduce your project by outlining its focus, aims, and significance within the current Health and Social Care sector.
  •  Develop a project plan that identifies and seeks to improve a specific area of practice, demonstrating your role and professional development as a Health and Social Care practitioner.
  • Explain how your project supports your professional growth, particularly in relation to building individual competencies and leadership qualities.
  • Appraise your current competence and leadership abilities, setting three SMART goals to guide your development and measure progress.
  • Reflect on your learning by evaluating what you have gained from completing the project—both in terms of your professional skills and your development as a leader.

Your Portfolio will serve as evidence that you have undertaken the planned activities and achieved the SMART goals established in your Learning Agreement (available on Moodle in the assignment information section). You should document your developmental journey on a weekly basis, detailing all activities that contribute to your SMART goals, including those not explicitly listed in your Learning Agreement. Each week, you should describe your progress, reflect on the specific actions taken, and evaluate how these actions have supported your ongoing development.

Suggested structure:

Introduction

  • Begin by examining an area of practice or a specific need within your workplace, community, or service setting. Analyse the situation to identify key challenges or opportunities for improvement. From this analysis, propose a range of possible solutions that could lead to realistic and meaningful enhancements in practice. Finally, justify your chosen project, explaining clearly why it represents the most appropriate and effective response to the identified need and outlining the product you intend to create as part of this work.

Analyse the competencies and leadership qualities required to successfully deliver this project. Discuss the competencies you expect to develop, evaluating which will be most beneficial to the project and most realistic to achieve within its scope.

  • How it relates to where I am now and where I aspire to be as a leader.
  • The skills I will develop through the project that relate to competency development for a role in health and social care
  • Sources of support including my Workplace Supervisor (if appropriate)

Create a detailed action plan of the project including steps, timescales and resources. Explain why each step is needed and how you will measure whether it worked.
