Learning Outcomes

• Demonstrate the following knowledge and understanding
• Critically review and evaluate your understanding of illness and long-term conditions, incorporating an awareness of the possible causation, impact and interventions related to these
• Create a 15-minute presentation in a format of your choice in which you evaluate integrated care in relation to a patient from practice.’
• You will identify what integrated care is, explore the national and local policies and structures that are in place to support this and critically evaluate how this has been utilised in the care of your patient.
• Your presentation must be no more than 15 minutes in length (+10% = 16.5mins). It must be clear for others to understand and be supported by current, relevant evidence and referenced and cited correctly. It can be in any form of your choosing but you must be able to present it in person in class.
• Explore what integrated care is using national and local policies to support this
• Critically evaluate this about the care provided to your chosen patient.
• This unit will enable you to respond to the physical, social and psychological needs of individuals. You will plan, deliver and evaluate, safe, competent, person-centred care paying specific attention to changing health needs during different life stages, including progressive illness and death, loss and bereavement. You will learn how to use a reflective model to develop on your professional practice.
• The unit addresses the central question: What health issues may affect individuals and how can your assessment and delivery of care optimise their recovery or management?
