5N4325 Assessment Brief 2026

As part of this assessment, learners are required write five individual reports on how they would undertake the following clinical skills:

1. Measurement and recording of temperature.
2. Measurement and recording of pulse.
3. Measurement and recording of respiration.
4. Measurement and recording of blood pressure.
5. Measurement and recording of fluid balance.

Evidence of the above will be provided by:

1. A written report, addressing the guidelines below.
2. An Observation report signed by supervisor. {See Below}
3. Completed observation charts

Guidelines

• Word Count: 800 words per skills report (+/- 10%)

Please note: if you do go over your word count deduction will be at tutor discretion, based on the relevance of the information submitted.

• Please see below instructions for guidance on structure and elements to be addressed in each of the reports

Any results issued are provisional and subject to confirmation from the QQI External Authenticator.

General Guidelines

• Your skill reports must be written in first person. Outlining how you performed the required skills.
• In your reports it is important to emphasize how you promote good client care, knowledge and understanding.
• When writing reports, it is also of great importance to use the language of the vocational/profession and avoiding ‘layman’ terms and inappropriate/unprofessional terms.
• The reports do not require research / referenced material.

Report Structure

the below structure must be followed. 

PREPARATION OF CLIENT: In this section outline how you  prepare your client for the measurement and recording of the skill.

ORGANISATION OF MATERIAL/RESOURCES REQUIRED: In this section outline how you  prepare all your equipment/material for the task.

PROCEDURE AND RATIONALE: In this section provide a detailed account of how you    undertake the required clinical skills, addressing the steps of the procedure in detail and rationale for interventions, outlining good practice.

SAFETY & INFECTION CONTROL: In this section outline the safety measures and infection control measures you  apply in safely executing the task.

REFELCTION ON LEARNING: In this section reflect on knowledge gained and identify how this new learning will assist you in practice going forward.

For final presentation of your work please ensure:

• Accuracy of information supplied.
• Written in correct context as instructed.
• Quality of Presentation
• Grammatical correctness and proper spelling
• Professional vocational language is used.

 Please note failure to adhere to the above, may result in deduction of marks.
