This Assignment will provide evidence of Learning Outcomes 6 & 9.

5N3769 GUIDELINES:

You must read the case studies in Chapter 2 of the book (Maureen p21, Noel p27 and Anouk p30). You are part of their team of carers who are attending to their ADLs. Choose 1 person to base your assignment on and complete the following assignment.

• Describe the person in your case study. (you may add to or tailor the case study)
• Describe how you will care for this client in a person centred way, in the last weeks of their life.Use the following headings –
  Physically, (Including Safety and Comfort)
• Emotionally and psychologically, (Including Dignity and respect)
• Socially
• Spiritually, (including assisting the client to deal with Grief, Bereavement and Loss)

Explain the skills that are required by a palliative support worker to care for this person or any person receiving palliative care.Use the following headings-

• Communication
• Pain management
• Care in the last 48 hours
• Care after death
• Ethics
• Self-reflection

All work must be submitted in a soft folder with a bar, no clip folders.

Evidence for this assessment technique may take the form of written, oral, audio, visual and/or digital evidence. Any audio, video or digital evidence must be provided in a suitable format.

Used the range of skills required from a palliative support worker in relation to end of life care.
