Assignment Requirements

You are required to complete all sections of the Care Support Module Brief.

These are detailed below:

Part A: This contains an assignment that requires you to conduct a client/resident observation and use your experiences of working in a healthcare setting to write a report using a topic that is listed in the brief.

Ideally, the topics you choose to discuss should be relevant to your chosen field of study and can be used to answer each question in the assignment brief. Your work should show evidence of relevant reading around each topic. Your work must be referenced and include a bibliography.

Part Two: This contains a Learner Brief, which requires you to reflect on your practice and outline the key areas of your learning. You are required to use the Gibbs reflection cycle you will provide a written account of each task. These can be based on your past experiences working in healthcare.

Assignment (30%) – Word Count 2000 words (+/- 10%)

Assessment Activity Description and Instructions to Learner:

You are required to conduct a resident/client observation.

For this task, you are required to observe the resident and write an observational report using a topic from the list below.

1. Social isolation/withdrawal
2. Emotional upset
3. Communication difficulties
4. Memory loss
5. Physical disability.

Observe the client when he/she is conducting an activity or is involved in a situation that poses difficulties for him/her.
Record your findings objectively under the following headings:

1. Client profile – Provide detailed background information, e.g., age, living in a care home/independently, mobility, illness/condition, family support or alone, medication, etc. (3 marks)

2. Provide a detailed account of the challenges experienced by the client about the chosen category above (3 marks)

3. Note the areas in which he/she demonstrated competence (what they can do well) and confidence. (3 marks)

4. Describe the measures/interventions that are presently being employed to meet the client’s needs. (4 marks)

5. Provide realistic suggestions to improve the quality of life of the client by:

  • making suggestions of actions or interventions the MDT could make (4 marks)
  • explaining what contribution you can make as an individual to the client’s wellbeing and quality of life. (4 marks)

6. In carrying out this assessment, outline your learning from a personal and professional perspective (6 marks)

7. Marks will also be allocated for the following:

  • Demonstrate evidence of critical thinking, additional research and correct referencing (2 Marks)
  • Formatting of project and overall layout (1 mark)

Learner Record 70% 2,500-3,500 words in total (+/- 10 %)
Each Reflection – 10 marks, 500 words approx. each (+/- 10 %)

Assessment Activity Description and Instructions to Learner:

You are required to reflect on your practice as a care worker and key areas of personal learning. You should base your reflections on past experiences when working in healthcare. You must use a reflective tool when writing the learner record, such as Gibbs Reflective Cycle. See an example of a reflective cycle template below. You should address each of the following areas:

1. Reflection on relationships that develop with clients, relatives and members of the health care team (10 marks)

2. Reflection on interpersonal issues that arose in care work, including the use of positive communication strategies (10 marks)

3. Reflection on your effectiveness as a care worker, identification of personal strengths and weaknesses (10 marks)

4. Reflection on the experience of receiving guidance and direction and giving and receiving feedback (10 marks)

5. Reflection on how you contributed positively as a team member through appropriate reporting and record-keeping procedures (10 marks)

6. Reflection on your awareness of client dignity and how you promoted the client’s Dignity (10 marks)

7. Reflection on the role and responsibilities of a care worker generally and how your beliefs and values impact your work as a care worker (10 marks)

Reflective Cycle Template Example –

Take the above 7 headings and apply each to the following reflective cycle or a similar reflective cycle:

  • Description of events – What happened?
  • Review – thoughts, feelings, what was good/ bad – what was difficult/ went well?
  • Evaluation – What have I learned? What influenced my actions? What were the consequences of the event (for me, the resident(s) or staff involved)?
  • Is there anything else I could have done? Could I have been better prepared? What skills do I need to acquire? Are there other people or resources I should have used? Would the outcome have been different if I had used resources or acted differently?
  • Future Plan/Action – If it happened again, what would I do? What do I now need to do? What timeframe do I need to do it? What have I learned about myself? What have I learned from this that I can take with me for the future?

Marking Guide

All marks are clearly outlined for each part of a question for your information when completing your assignment brief. Please follow the marking guide when planning your
answers.

Structuring your Assignment

Examine the marking scheme for each assignment, and use the headings provided in the marking scheme as paragraph headings for your work. By doing this, you are far less likely to leave out something that attracts marks and also stops you from focusing too much on one area (One of the most common ways to lose marks).

Things to watch out for

  • Marks do not carry over from one section to another. No matter how much you write on a given section, you cannot score more than the maximum allowed marks for that section.
  • If you do not use headings in line with those in the marking guide, you are far more likely to leave out something that’s worth marks. Headings also make it easier for the examiner to give you marks, rather than having to hunt through an assignment for mention of the topic.
  • Get somebody else to proofread your work before you submit it. It is much easier to spot mistakes in another person’s writing, and you can lose marks for careless mistakes. Tutors recommend that you send a draft of your assignment for feedback before submitting it in full to ensure you are heading in the right direction.
  • Do not be significantly under or over the required word count (10% above or below is ok).
  • Formatting matters! Use paragraphs and put headings in bold. Only use fonts such as Times New Roman or Arial as they are both professional and clear to read. Change the vertical spacing on your document to 1.5, only use black font colour and use page numbers to make your assignment clearer to the assessor.

A record of proof of your required work placement hours.

Submitting your Work

You must follow these guidelines when you are submitting your work

Before you submit your work, you must ascertain that all aspects of the assignment brief have been answered. Organise your Documents to ensure you have not missed anything. A list of required documents is included in this booklet (Pages 13-14). Failure to follow this will result in your work not being graded until all aspects of the brief are submitted, and could potentially cause a delay in results being issued. Please remember that Chevron Policy dictates that Partial Submissions (All parts of the Assignment Brief have not been submitted in full) will not be accepted. To ensure you are submitting correctly, please upload all documents on the same day.

All written work must be submitted to the Turn-It-In Portal as per Chevron Policy, with no exceptions. A Turn-It-In Tutorial has been provided to you under the session ‘Submitting to Turn It In and Class IDs’ on your Learn Upon dashboard.
