Question 1a (250 words, 20 marks) 

Mr. Lim is a 59 year old Singaporean Chinese male, BMI 30 kg/m2, diagnosed with Type 2 Diabetes (T2DM) three years ago. He works as a logistics supervisor and often does shift work, including night shifts. He lives with his wife and two adult children in a 4-room HDB flat. 

  • At his recent polyclinic check-up,his HbA1c was 9%, up from 7.6% a year ago. He reports feeling increasingly fatigued and frequently wakes up at night to urinate.
  • he gained 5 kg over the past 6 months.
  • his blood pressure is high (150/90 mmHg). He has been prescribed metformin and a low-dose statin but admits that he sometimes forgets to take his medication during his night shifts.

Mr. Lim states that he finds it hard to eat healthily because he often eats late at night at coffee shops near his workplace. He does not engage in regular physical activity, claiming that he’s “too tired after work.” He also frequently drinks redbull and enjoys kaya toast everyday for breakfast. When asked about his stress levels, he shares that his work is increasingly demanding and he has little time for himself.

Despite these challenges, Mr. Lim says he wants to “get healthier” because his father recently had a stroke, and he is “scared the same thing will happen” to him.

Having identified areas of concern in Mr. Lim’s lifestyle and clinical markers, explain what are some possible complications that may rise if Mr Lim does not make changes to his lifestyle.

Question 1b (350 words, 30 marks)

Recommend a strategy or intervention that is evidence based and realistic for Mr. Tan’s context that can address the areas of concern. Support your strategies using credible clinical guidelines or literature (e.g. MOH Clinical Practice Guidelines, WHO diabetes prevention resources).

  • You need  not do  an  intervention to  address  all his concerns.  You may pick and choose the more important ones.  

Question 2 (700 words, 50 marks)

Imagine yourself 20 years into the future where you have worked on your current health  Issues/concerns. Write a letter to your current self. Assess your health status and personality style and apply the transtheoretical model of health behaviour change to evaluate how you can commit to making this change.

Demonstrate your ability to be an effective health and wellness coach to your current self by encouraging yourself to work towards sustaining a healthy lifestyle.

Additional guidance:

  • The assumption is that 20 years from now, you are more aware of your health and how your current health practices may influence your health.
  • Specifically use the Transtheoretical Model in your answer to identify your current stage of change and what are some of the practical and specific steps you can commit to in order to move from your current stage to the next.

Appendix A: TMA/GBA Submission Reminders  

TMA/GBA Cover Page & GBA Declaration 

  • Download and complete the TMA/GBA Cover Page from Canvas L01 > Modules > Assessments.
  • Acknowledge that you have read the self-declaration on academic integrity and submit it  with  your  TMA/GBA.  There  will  be  a  penalty  of  5  marks  deduction  if  the TMA/GBA cover page is incomplete, inaccurate or missing from the final submission of  your  TMA/GBA  via  Turnitin.  No  appeals  concerning  the  Cover  Page  will  be entertained.

TMA/GBA submission  

o Adhere  strictly  to  the  naming  convention  specified  for  TMA/GBA  files.  Non-compliance to these standards may result in your TMA/GBA not being marked and/or an administrative fee being imposed. o It is your responsibility to ensure that you have uploaded your file(s) completely and accurately to the correct submission link. All inaccurate submissions will be treated as a null submission.  o Backup your TMA/GBA at all times. o Review your submission immediately  and ensure that  the  entire  document  has been uploaded completely and successfully. o Submit  early  and  check  the  originality  report  to  decide  if  you  need  to  revise  and resubmit your assignment.

  • Turnitin reports are generated immediately the first time but subsequent reports may take up to a day to generate. 
  • Retain  the  Turnitin  digital  receipt  as  evidence  of  a  successful  submission.  Appeals submitted without the Turnitin digital receipt will not be entertained.

  • If there was no submission before the deadline, one late submission is allowed up till 1 week after the deadline, after which no more late submissions will be allowed.

TMA/GBA Formatting Requirements [For essay submissions on Word only] 

  • Upload your TMA/GBA as a Word document file (.doc, .docx) only. No other formats are allowed. Feedback on your marked assignments will be provided only if the assignment is submitted as a Word document file.   
  • Use 12-point Times New Roman and double line spacing throughout the entire paper. 

Margins: 1 inch (2.54 cm) margins on all sides (top, bottom, left, and right) of the page 
Line spacing: Double-spacing 
Paragraph alignment: Align the text to the left and leave the right margin uneven 
Paragraph indentation: Indent the first line of every paragraph 0.5 inch 

Word / Slides / Page / Duration Limit 

Writing and presenting succinctly is an important skill in academic writing. English Competency

Plagiarism and Collusion 

The University takes a very serious view of plagiarism (passing off someone else’s ideas as your  own;  recycling  of  contents  from  your  own  earlier  marked  TMA/GBA  from  the  same course  or  another  course;  or  passing  off  AI-generated  content  as  your  own)  and  collusion (submitting  an  assignment  which  is  the  same  or  very  similar  to  another  student’s).  The definitions and penalties of plagiarism are outlined in your Student Handbook under Academic Rules and Regulations. All assignments are to be completed on your own. You may discuss the TMA/GBA with your course-mates, but the assignment must be written up independently. Do not share your notes, draft or final TMA/GBA with anyone before the marked TMA/GBAs are returned to you. You are strongly advised to submit your TMA/GBA early and review the plagiarism report yourself, and if needed, revise and resubmit your TMA/GBA before the submission deadline.

Avoid plagiarism by giving yourself sufficient time to research and understand the material so that you can write up your assignment in your own words. Quotations should be used sparingly.
